UPMC and GECAC want to provide community support services to help seniors maintain and preserve their independence to direct their own care.

Our Impact

GECAC provides person-centered care to approximatly 100 seniors through the Community-Based Care Coordination Initiative in partnership with UPMC.

Overview

GECAC has developed a partnership with UPMC called Community-Based Care Coordination Initiative (CBCCI). Seniors receiving care management in their homes through GECAC that are also insured by UPMC for Life will receive additional benefits.

These benefits include:

  • Over the Counter Wellness Benefit
  • Meals following a hospital discharge
  • Bathroom Safety Items
  • Personal Emergency Response Systems
  • Automated Medication Dispenser
  • Transportation
  • In-Home Safety Assessment
  • Health and Wellness
  • Podiatry
  • Dental

Enrollment

  • Age 60 or older.
  • Determined to be in need of services in order to maintain independence.
  • Have UPMC health insurance.
