Our Impact

Nearly 400 low-income people living in the Housing Authority of the City of Erie's Schmid Towers and Friendship Apartments buildings are able to maintain their independence.

Overview

HomePLUS reduces risk factors and stabilizes housing by providing assessment, case management, and other aging services on-site to Housing Authority of the City of Erie's residents at:

Schmid Towers
153 East 6th Street

Friendship Apartments
111 East 11th Street

LECOM/LifeWorks provides medical case management and support services for the HomePLUS program.

Enrollment

  • A resident of Friendship Apartments or Schmid Towers.
