Position Summary
Applications are being accepted for a full-time Caseworker position for the HomePlus Program in the Area Agency on Aging at GECAC. Provide direct services to Erie Housing Authority apartment clients to assist in the identification and resolution of problem areas in maintaining independence. Facilitates the use of community resources and services. Establish and periodically reassess care plans, making modifications as needed. This position will work Monday-Friday from 8:30 AM to 4:30 PM.
Key Responsibilities
- Informs clients of available services and develops with clients a care plan designated to meet their needs.
- Monitor services for each client through face-to-face contact to assess the adequacy of service delivery and coordinate services.
- Sends necessary information to service providers to ensure the promptest possible service delivery.
- Utilizes and coordinates community resources to assist the client in activities of daily living.
- Documents and maintains client records as required in line with established procedures and completes reports monthly.
Qualifications
- Bachelor's Degree in psychology, gerontology, or other behavioral sciences; and directly related experience. Qualifying experience would include coordinating assigned services as part of the clients' care or treatment plan, teaching clients living skills, aiding in therapeutic activities and providing socialization opportunities for clients. Experience providing socialization opportunities for clients. Experience providing hands-on personal care for disabled or elderly clients, maintenance of the client's home, room, or environment, or aiding in adapting the physical facilities of the clients ' homes not qualify.
- Knowledge of Microsoft Office (Outlook, Word, Excel, and PowerPoint) and Microsoft Teams. The ability to operate a computer in a Windows 10 environment, Familiarity with Zoom, PDF editing software, and experience effectively working remotely are desired. Enough knowledge to successfully complete the technical skills of the position.
- Act 33, 34, and FBI Clearances issued within the past 5 years.
- Valid driver's license and full-time access to a private automobile
