Monday – Friday 8:30 – 4:30 (7.5 hours daily)
Applications are being accepted for a full-time case worker position for the Assessment Program in the Area Agency on Aging at GECAC under contract with Aging Well of Pennsylvania.
Qualified applicants will provide direct services to consumers conducting Functional Eligibility Determinations (FED) required for Medicaid eligibility and answer inquiries regarding local resources.
Conducts initial functional assessments to determine problem areas and service requirements.. Provides direct services to clients who have applied for institutional long-term care, for in-home community-based long-term services and supports, and clients in community residential care as needed. Documents Functional eligibility using a standardized form and updates the statewide database, which leads tothe level of care required by clients. Determines the degree of informal support available to the client. Maintains information and facilitates the use of resources available in the community. Maintains an information and referral system with regard to answering inquiries and maintaining current information.
Must have good communication and outreach skills, and have knowledge of community resources. Also, must have the ability to document and maintain client records and complete monthly reports in line with established procedures. Ability to speak or read languages other than English, especially Spanish or Russian, is desirable.
REQUIRED: Bachelor's degree with 12 credit hours in sociology, social work, social welfare, psychology, gerontology, or other related social sciences. The position requires two years of casework experience, including one year of assessment experience. OR one year assessment experience and a Bachelor's degree with a social welfare major OR any equivalent combination of experience or training, including completion of 12 semester credit hours of college-level courses in sociology, social welfare, psychology, gerontology, or other related social sciences. One year of experience in the AAA system may be substituted for one year of assessment experience. Proficient in computer use and have computer software skills, including Microsoft Office software.
Budgeted at $21.46-$22.56 per hour starting, depending on prior experience, with an excellent benefits package.
