PDA Waiver Program
The PDA Waiver Program offers eligible participants an alternative to institutional care. For those eligible clients/consumers, who have been assessed with a nursing home level of care and determined financially in need by the DPW/CAO, case management staff develops an individual service plan and arranges for a variety of in-home services to insure the participants can safely remain in a community setting. Benefit counseling is made available to those consumers that may experience difficulty in pursuing financial eligibility for medical assistance to qualify for this program.
Provides services to those persons 60 years of age or older who meet all of the following requirements:
- Meet Medicaid financial requirement
- Require the level of care of a nursing home
- Choose community-based services rather than a nursing home
- Can be safely served in the community based on a care plan of services approved by the state
Adult Daily Living Services, Home Health Care, Home Support, Personal Care, Personal Emergency Response Systems (PERS), TeleCare, Counseling, Financial Management Services (FMS), Participant Dircted Community Supports, Participant Directed Goods and Services, Community Transition Services, Personal Assistance Services, Companion Care, Respite Care, Home Delivered Meals, Specialized Medical Equipment and Supplies, Home Modification, and Transportation.
Follow Up Period: Monthly as determined by an individual contact plan. Reassessments are done annually, unless determined to need it sooner.
