The HomePLUS Project is designed to reduce risk factors and to stabilize housing by providing assessment, case management, including mental health services, and other aging services on-site to Housing Authority of the City of Erie residents at Schmid Towers and Friendship Apartments. Partner Agency (Erie Center on Health and Aging) provides medical services.

Program Location:

Schmid Towers – 153 East 6th Street, Erie (814) 456-1511
Friendship Apartments – 111 East 11th Street, Erie (814) 456-8806

Participation Requirements: 

Completion of comprehensive assessment upon move in to the building.

Follow-up period: Participants are monitored monthly as needed. Clients are reassessed annually or as needed (every six months).
