Applications are being accepted for a full-time Mental Health Caseworker position for the Home Plus Program in the Area Agency on Aging at GECAC.
Qualified applicants will provide direct Mental Health and case management services to Erie Housing Authority apartment residents to assist in the identification and resolution of problem areas in maintaining independence. Facilitates the use of community resources and services. Establish and periodically reassess care plans making modifications as needed.
REQUIRED: Position requires a Bachelor's degree in sociology, social work/social welfare, psychology, gerontology or other of the behavioral sciences and four (4) years experience. Qualifying experience would include coordinating assigned services as part of the clients' care or treatment plans, teaching clients living skills, aiding in therapeutic activities and providing socialization opportunities for clients. Experience providing socialization opportunities for clients. Experience providing hands-on personal care for disabled or elderly clients, maintenance of the client's home, room or environment or aiding in adapting the physical facilities of the clients' homes not qualifying. Proficient in the use of the computer and have computer software skills including Microsoft Office software. Good communication skills both verbal and written. Good outreach skills and knowledge of community resources and funding source regulations needed. Must have a valid PA driver's license with full-time access to private automobile and auto insurance as required by the Agency. Must meet Immigration Law requirements. Must provide Act 33 & 34 and FBI Clearances issued within the past year. The budgeted range at $15.04-$20.05 per hour starting, depending on prior experience, with an excellent benefits package.
