The “Erie County Link” to Aging and Disability Resources is designed to be a coordinated network of Core and Collaborative Partner agencies which allows for the consumer to initiate contact with any Link Network partner and, generally speaking, experience the same intake to access information, assistance and resources.
The Administration on Aging and the Center for Medicare and Medicaid Services (CMS) launched the Aging and Disability Resource Center (ADRC) grant initiative to promote the integration of long-term care information and referral services, benefits and options counseling services, and access to publicly and privately financed services and benefits for those in need of long-term supports and their families.
Assists the nursing home resident with transition back to community living. The Long Term Living Counselor advises the participant about community programs and resources and assists the participant in making an informed decision to leave the nursing home. The NHT care manager then works with the participant and the nursing home to develop a plan for successful discharge.
A consumer directed model of care that provides for attendant support for personal care, home support, and other services as needed by the participant to maintain independent living in their community setting.
