Adult Day Care . . .

   
 

Subsidized day care which provides a structured, secure environment with daily activities and programs.

  

 

 

Adult Protective . . .

  
 

Provides 24-hour access to case workers who investigate abuse, neglect, exploitation, and abandonment of persons 60 years of age and older.  Guardianship and power of attorney services are also provided.

 

 

Attendant Care . . .

 

 

 

Assistance with bathing, dressing, feeding, ambulation, housekeeping, and meal preparation for physically impaired seniors.

 

 

 

Care Management . . .

   

A trained care manager will assess client needs, arrange for services, and monitor services on an on-going basis.

 

 
   
 

Community Care Services . . .

  

Provides in-home and community services, on a limited basis, to persons 18 years of age and older who would qualify for nursing home care but whose care needs can safely be met in the home at substantially less cost to the Commonwealth than nursing home care.

   
 

Domiciliary Care . . .

  

Alternative living arrangements for persons unable to live independently. Private family homes are certified as providers. Clients are placed and monitored by case managers.

 

 
 

Family Caregiver Support Program . . .

   
 

Provides assistance to caregivers caring for a loved one at home.  Assistance includes care management, education, benefits counseling and financial reimbursement for the costs association with paying for in-home services and medical supplies.  In addition, reimbursement for the purchase of authorized equipment and home modifications may be provided.  The Kinship Care Portion of this Program, assists caregivers aged 60 years and older, who are raising a child 18 years or younger and is identified as being at-risk.  All reimbursements through this Program are contingent upon prior approval, determined through an assessment process.

   
 

Foster Grandparent Program . . .

   
 

Provides eligible persons, 60 years and older, with a modest stipend for volunteering in specified settings with young children who have special needs.

   
 

Home Health . . .

   
 

Registered nursing services not covered by Medicare may be covered in the home.

   
 

Home Support . . .

   
 

Assistance with laundry, light housekeeping, meal preparation, and grocery shopping.

   
 

HomePlus . . .

   
 

Supported through the Housing Authority of the City of Erie and coordinated with GECAC, the Erie Center on Health & Aging, and Stairways - The program offers care management, medical services, and emotional support to select senior high rises.

Information Assistance . . .

   
 

Serves as the entry point for AAA services. When a referral is made, a caseworker assesses a client for eligibility for AAA services or refers the client to the appropriate community resources including Home and Community Based Services such as Attendant Care, Aging Waiver, and the LIFE Program http://www.lifenwpa.org.

   
 

Meals On Wheels . . .

   
 

A hot noon meal delivered to homebound seniors.

   
 

Ombudsman . . .

   
 

Advocates for residents of long-term care facilities. Evaluates and mediates the complaints on behalf of such residents.

   
 

Personal Care . . .

   
 

Assists with bathing, dressing, grooming, and some light housekeeping.

   
 

Pre-Admission Assessment . . .

   
 

Assessments performed on those individuals 18 years and older who are seeking placement in nursing homes, personal care homes, or community group homes; to determine appropriate level of care.  Phone:  (814) 459-4581  Ext. 400 or 480 or Toll free  1-800-769-2436

   
 

Senior Advocate . . .

   
 

Conducts activities to make older adults aware of services and benefits available to them.  Represents the needs and interests of older adults to government and community.  Assists individuals with problems in obtaining benefits and services.

   
 

Waiver . . .

   
  Provides services to those persons 60 years of age or older who meet all of the following requirements:
  • Meet Medicaid financial requirement

  • Require the level of care of a nursing home

  • Choose community-based services rather than a nursing home

  • Can be safely served in the community within budgetary guidelines

Services provided include:  Home health services (physical therapy, speech therapy, occupational therapy, nursing), Physical consultant, emergency response systems, counseling, Personal Assistance Services, personal care, home support, companion care, respite care, adult day care, home delivered meals, medical supplies and equipment, home modifications, and transportation.
